I certify that the facts contained in my Application are true and complete and that, if I am employed, falsified statements on my Application shall be grounds for dismissal.

 

I authorize investigation of all statements in my Application, and for you to contact any references and employers listed therein to obtain any and all information concerning my previous employment and any pertinent information that may have, personal or otherwise, and release the company from all the liability for any damage that may result from utilization of such information.

 

I also understand and agree that no representative of the company has any authority to enter into any agreement for employment for any specified period of time or to make any agreement contrary to the foregoing unless it is in writing and signed by an authorized company representative. If employed, I understand I am an at-will employee.

 

This waiver does not permit the release or use of disability-related, genetic or medical information in a manner prohibited by federal and state laws.

 

CONSENT TO BACKGROUND CHECK

I hereby freely and voluntarily give consent to a background check including but not limited to criminal and validation of driver’s license and driving record history, the results of which may be forwarded to any future employer. I declare that I have not been coerced in any manner whatsoever to sign this consent form and I understand that I have a legal right to refuse to submit to this test as a condition of new or continued employment.

CONSENT TO DRUG SCREEN

I hereby freely and voluntarily give consent to a drug screen, the results of which will be forwarded to any future employer. I declare that I have not been coerced in any manner whatsoever to sign this consent form and I understand that I have a legal right to refuse to submit to this test as a condition of new or continued employment.

GLOBAL TECHNICAL RECRUITERS EMPLOYMENT AGREEMENT

Global Technical Recruiters requires that if you are employed through our company that you, you will give Global 48 hour notice for any reason a job assignment does not work out and you choose to leave the assignment. If you fail to give 48 hour notice, your remaining hours that have not been paid will be reduced to minimum wage.

STANDARD POLICY FOR INJURED EMPLOYEES

Any Global Technical Recruiter employee that is injured while at work must follow the following policy

  • Report the injury to your supervisor immediately
  • Report the injury to Global Technical Recruiters as soon as possible
  • Fill out an injury report at Global Technical Recruiters within 24 hours of the time of the injury
  • Submit to a drug test within 24 hours after filling out the injury report
  • Employee must inform Global Technical Recruiters when they are cleared to return back to work and/or must provide documentation of how long employee is not permitted to work or work with restrictions.
